Using the $pageHeader option on the configuration page
Scratched my head on this one for a while... the $pageHeader option on the config page is literally what it says, "An HTML fragment inserted at the top of each page." NOT a reference to a local html document, like an SSI. Therefore, take your html, and copy/paste it into that itty bitty field, and it'll work. Took me about half an hour to figure that one out, hope this helps someone else...
For large $pageHeaders, you can now use a string like 'file=foo.bar', and the file foo.bar will be included verbatim from your fom-meta directory.
After you've changed the header or footer contents, you may want to rebuild all the cached files.

Go to the configuration menu (at /cgi-bin/fom?cmd=install) and use the "rebuild cache and dependency files" option.
